You will need a valid State or Federal Photo ID and a test authorization form. Frequently your employer will provide an authorization form. If you are doing personal testing all you will need it the ID.
Usual this is not a good choice. Depending upon who you are doing the test for will determine the consequences. If it is a Federal Mandated Test, you will be required to complete an approved Substance Abuse Program and take subsequent Observed Tests until released by you SAP. If the test is for Family Law, you may loose visitation privileged to your children. If it is for your Probation Officer, you may end up back in jail. Be sure you know what happens in your case if you choose to refuse a test. Once you leave the Office Lobby, we are not able to “fix” a refusal to test.
